37. **** Facility has several working containers of sauces and spice blends that do not have the names displayed on them. Ensure that these are labeled with the name of the food. ****

49. **** The pizza maker has a build up of debris on the back side of it, ensure that this is cleaned regularly to avoid accumulations. ****

51. **** The grease trap has a build-up of debris that appears to be leaking from the trap as well as an odor emitting from it. The PIC states that it is getting cleaned today and that it is getting replaced, but the facility is waiting on a part. Ensure that this is cleaned and maintained in good repair. ****

55. **** There are several areas where the floor is chipped, exposing the floor beneath it. Ensure that these are repaired so that the floor is smooth and easily cleanable. ***

49. Observed non-food contact surfaces with a build up of debris. The exterior of the pizza ovens and other equipment, the wall surfaces near the pizza oven and area where the floors meet the walls should be cleaned more frequently to avoid accumulation of food debris and/or dirt.
