56. Ventilation grill on ceiling of bakery was heavily soiled with dust and debris. Possible paint peeling from grill. Clean/replace grill so that debris is not falling onto food contact surfaces. Replace filter behind grill as soon as possible.

55. Dishroom ceiling tiles are heavily stained, and not easily cleanable due to porosity. Replace dishroom ceiling tiles. Ceiling vents and light fixtures need cleaned, repaired or replaced due to rusting and a build up of debris.

54. The handwashing sink by the the ovens and in the serving area did not have trash cans. Ensure each hand sink has an accessible trash can by next routine insepction.

51. The hand sink by Mark's station and in the dish washing area were leaking. The ice machine had a small container under a leak. Have a licensed plumber fix these areas by next routine inspection.

38. The door by the office to the dumpster has weather stripping that has come out of place, so the door is not sealed properly. Daylight is able to be seen through the crack between the door and the frame. PIC states this problem has been fixed multiple times, the door gets hit and the problem returns. To keep pests out, doors should have no holes and no light should be seen through cracks. Ensure there is no crack between the door by next routine inspection.

28. A pitcher of an unknown liquid was found near a chemical spray bottle. A bottle of spray cleaner was found on a food prep area. *COS* The cleaners were moved to an appropriate storage location. Ensure toxic substances are stored away from food and food prep areas.

23. A pan of meatballs was found labelled with 2-28 was found in the walk in cooler. According to this label, this food should have been discarded one week ago. A container of salami was found in the walk in with a label of 2/23. According to this label the salami should have been thrown out two weeks ago. A pitcher of alfredo sauce was found with a label of 2/14. According to this label, this should have been discarded 3 weeks ago. This is a repeat violation. A risk control plan was filled out with PIC. *COS* The food mentioned was discarded.

15. A large carton of unpasteurized eggs was found next to lettuce in a box and ready to eat meat balls in the walk in cooler. *COS The eggs were moved to a bottom shelf away from these foods.
