37. Several large totes of dry food products (several are white in color) are found at the back of the kitchen without a label of their common name. Label all food products when they have been taken from their original container so they aren't misused.
55. 1. The corner and upper wall near the back exit door is soiled. 2. The vent on ceiling in the hallway which leads to the back of the kitchen is heavily soiled. Maintain all ceilings and attached equipment clean.
56. 1. The light in the back of the kitchen above the dry storage is burnt out. 2. The light in the hallway which leads to the back of the kitchen is burnt out. Maintain adequate lighting.

39. A bag of onions are found to be stored directly on the floor in the walk-in cooler, some containers of raw beef and chicken were found in the walk-in cooler uncovered, and personal food items were intermingled with food items for the customers in the walk-in cooler. Food shall be protected at all times. Elevate the onions up and off the floor, cover all food items during storage and label and separate the personal food items for food protection.
Corrected on site by elevating the onions and covering the food products. Also, the personal food items were relocated and labeled.
49. The large white deep freeze, located at the end of the cookline, across from the hand sink and the woks, had blood built-up around the inner top, just below the lid. Keep on non-food contact surfaces clean at all times. The freezer has been cleaned on site.

15. Observed raw pork above raw shrimp in the walk in cooler. COS, the pork was moved below the shrimp.
43. Observed 2 bowls being used to scoop food and more than one scoop stored with handle in food item. COS, bowls were replaced and scoops were corrected after discussing with the PIC.
49. Observed multiple areas in the facility that need cleaned more frequently. Walls, floors, equipment and under equipment should be cleaned at a frequency necessary to preclude accumulation of food and dirt residue.
