10. Hand sink in the bar and accessible bathroom do not have hand washing signs. The men's room handwashing sign has been defaced so it is unreadable. *** COS Inspector gave employee signs and instructed they be posted.
16. The microwave cavity was soiled when no food service had been started for the day. *** Increasing cleaning of the microwave to every 24 hours before next routine inspection.
22. The sliding door cooler was holding around 44 deg F, and one prep table was holding around 63 deg F. This is likely due to standing water in facility. *** Manager threw milk, deli meat, cheese, soup, sour cream, pork chop, garlic in water, queso, bacon, ham, wing sauce, garlic butter, chicken, and hamburger meat in the garbage. Employee put soap on all items so that they cannot be used. Inspector checked to see that the items were thrown in the dumpster before the end of the inspection. All items were above 41 deg F. Email inspector invoice of work done to remedy issue, whether that is work to coolers, or electrical work, by 4/3/25.
23. Found soup, ranch, and cheese with no date labeling. *** COS Soup was thrown out. Ranch and cheese were labeled.
38. Door leading outside in the kitchen has gaps between the jam and the door itsself. *** Replace the door with a tight fitting door before next routine inspection .
51. The grease trap and ice machine drain below the flood rim of the floor drain. *** Bring the drain lines of both above the flood rim of the floor drain by 4/3/25. Disconnect the ice machine, drain it, and wash, rinse, and sanitize it by 4/3/25. The ice cannot be used due to potential contamination.
51. *** The floor drain below the three compartment sink was overflowing, and the kitchen had stagnant water on the floor. *** The floor drain must be repaired by a licensed plumber. The kitchen must remain closed until the floor drain is repaired by a licensed plumber. Clean the floor of stagnant water. When this is completed, contact inspector, and then the health department will re-evaluate for reopening. Inspector will return on 4/3/25 to do an inspection regardless of getting contact from the facility. Facility informed inspector that they had a plumber coming that night, and a scheduled visit on Wednesday. Employee believes that the drain was intentionally and manually clogged. Inspector assured him that would be included on the report.
53. The accessible bathroom door is not self closing. *** Replace or make the door self closing.
55. The concrete floor in the kitchen has worn so that it is pitted. *** Flooring in food prep areas must be smooth and easily cleanable. Replace flooring. This violation was on the last inspection report as well.
55. Floors and walls, especially behind and under equipment have a build-up of debris. The pipes and underside of the sink has the most build-up. *** Increase the cleaning frequency of the facility before the next routine inspection.

57. *** All food handler certificates on file were expired. *** Obtain copies of food handler certificates, or ensure employees get food handler education.
55. *** The concrete floor in the kitchen has worn so that it is pitted. *** Flooring in food prep areas must be smooth and easily cleanable. Replace flooring.
55. *** Makeshift mat flooring has been installed so that it completely covers the floor of the bar area. *** Remove the mat flooring. Floor must be of smooth and easily cleanable material or appropriately installed carpet.
51. *** Makeshift grease trap was constructed and install in the bar area. *** The replacement grease trap was on the premises, however, it has not been installed yet. Have a licensed plumber install new grease trap by 9/16/24. Email me an invoice of work done by licensed plumber.
51. *** Observed a leak in the floor near makeshift grease trap. *** Have a licensed plumber look into and fix leak before 9/16/24. Email me an invoice of work done by licensed plumber.
49. *** Non-food contact surfaces such as handles of coolers, front of coolers, sides of stove, shelves, etc. throughout kitchen were greasy to the touch and visibly soiled. *** Clean surfaces before next routine inspection.
36. *** I could no locate thermometers in the sliding glass door coolers. *** Provide thermometers that are readily accessible inside those coolers by 9/16/24.
16. *** The lip of the prep table, stove top, and cooking utensils in the stove had a build up of debris. *** Wash, rinse, and sanitize all food contact surfaces at a frequency that prevents build-up of debris. Clean these items before follow-up inspection on 9/16/23.
10. *** Handwashing sink had alcohol bottles, a funnel, and towels inside of it. *** Owner moved the items out of the sink. Hand sinks must be accessible at all times.
5. *** Owner could not locate procedure. *** COS Inspector provided poster with clean up information.

55. The back tile behind grill has some cracked tiles. This could create a cleaning issue. It must be smooth durable and cleanable. Ensure this is corrected by next routine.
