2. **** Person in charge was not a CFPM. There was no CFPM available at time of inspection. A medium risk food facility must have a CFPM present during all hours of operation. This is a chronic repeat violation. Facility has 30 days to provide a schedule of times a CFPM will cover shifts as well as CFPM documentation to inspector. ****
3. **** PIC did not know the five reportable symptoms and could not locate an employee illness policy at time of inspection. COS, inspector provided form 1-B to PIC. ****
22. **** Line deli cooler was down during inspection, and measuring at room temperature. There were several TCS times stored in the cooler- hot dogs, homemade ranch, cups of pre-portioned chili, and cheese. As per PIC the ranch was just prepared and the cheese had come from downstairs refrigeration just after lunch. Temperatures of the cheese and ranch were in the 50s and supported this claim. The ranch and cheese were moved to a working fridge and the hot dogs and chili cups (which also were not dated, see below) were voluntarily discarded by the PIC. The Walk-in cooler as well as the under dessert cooler and self serve cake freezers were down. No TCS was being stored in these at time of inspection. Ensure that no TCS food is held in any of these coolers and that a receipt of repair is sent to the inspector.
23. **** Hot dogs and chili that was portioned in cups were held without date marks in the establishment- COS, the PIC discarded these (They were also being held at out of cold holding range, see above). ****
24. **** Facility was practicing time as a public health control with cheese not held under refrigeration without the proper documentation. COS, PIC moved the cheese to a cold holding unit. If facility is to use TAAPHC, they must have procedures and records to do so. COS. ****
38. **** The back door from the dinning room was not self closing and has sunlight coming through the bottom. Ensure that this door is made to be self closing and tight fitting to prevent any pests. ****
40. ***** Employee did not have an effective hair restraint while preparing food. Ensure that head coverings are worn during all food preparation. ****
49. **** The bottom of the chest freezer next to the counter had an accumulation of wrappers and debris. -The Hood vents above the fryer had an accumulation of grease. -The caramel dispenser had a build up of caramel on the top of it. Ensure that all of these areas area cleaned and cleaned regularly to prevent accumulations. ****
55. *** Several ceiling tiles have an water damage on them and in the basement several pieces of the ceiling are starting to rust. Ensure these tiles are replaced and maintain physical facilities in good repair. This is a repeat violation. ****

2. **** PIC was not a CFPM and no records of employee CFMPs could be located. Please email elusk@tchd.net with a copy of a current CFPM certificate and ensure that during all hours of food prep and service a CFPM is on site. ****
3. **** PIC was not a CFPM and could not list or locate a list of reportable symptoms. Inspector emailed form 1-B. ****
8. **** Employees did not wash hands in between glove changes. When notified by the inspector, employees washed hands between glove changes. Please ensure that hands are washed before donning gloves for working with food. ****
16. **** Carmel dispenser had an accumulation of caramel around the nozzle. Employees cleaned and sanitized the dispenser. Please ensure that food-contact surfaces are regularly cleaned and to be clean to sight and touch. COS. ****
40. **** Employees did not have on hats or hair nets are did not have any on site. Please ensure that head coverings are worn during all food preparation. ****
49. **** Several freezers in the basement have an accumulation of dirt and rust on the tops. Please clean and ensure that these are kept free of accumulations. Inspector will check these during next inspection. ****
51. **** Several areas of plumbing in the basement are actively leaking. There is a pipe when you first walk down, some leaking / standing water in under the stairs, and a large area of standing water by the furnace. This is being referred to the city of Pekin and will be followed up on. Please have repairs conducted by a licensed plumber and receipts kept and emailed to elusk@tchd.net. ****
55. **** Several areas had an accumulation of debris. Chiefly among these areas were the floor of the walk-in cooler, the stairs to the basement, and the area behind the fryers. Please ensure that these areas are routinely cleaned to avoid build-up. ****
55. **** Several ceiling tiles have an water damage on them, Please replace these tiles and maintain physical facilities in good repair. ****
55. **** The basement has several coolers and freezers that are not functioning in the basement. Please ensure that equipment that is no longer used is not kept on the premises and that in the meantime signs saying "not in use" are attached in order to ensure that no food is accidentally placed in them. ****
57. **** Employees on staff could not provide and did not have food handler's licenses. Please have employees obtain their licenses within 30 days and email copies of certification to elusk@tchd.net.
