22. The individual SunGlow butter containers were found on the table at the wait staff area out of refrigeration since 7am (5.5 hours) and the label states: "Perishable Keep Refrigerated". The label was pointed out the the employees and the owner. They said that this is a new type of butter than what they used to use and they didn't realize the this type is required to stay under refrigeration compared to the previous kind. All cold TCS (time/temperature control for safety) food items need to be kept at 41 degrees F or below at all times. Corrected on site by voluntarily disposing of the (about 30) individual containers and will keep the butter under refrigeration from this point forward.
55. The following items are soiled and need to be cleaned: 1. The vent above the ice machine. 2. The vent cover on the ceiling of the men's and women's restrooms. Maintain all ceilings and attached equipment clean.

16. The chlorine bleach sanitizer located in the bucket at the wait service area tested at above 200ppm. All chlorine bleach sanitizers used for food-contact surfaces shall test around 100ppm. The sanitizer has been diluted and now tests at 100ppm.
22. The Pepsi cooler located in the wait service area has an air temperature of 57 degrees F. The food items temped at 54-56 degrees F. The food items which are required to be refrigerated for food safety have been voluntarily disposed of. See the pictures attached for all food items which have been disposed of (such as ranch dressing, french dressing, thousand island dressing, coffee creamers, individual containers of butter, & whipped cream. The Person In Charge said the Pepsi company is sending a service technician out and they will be there tomorrow (5/15/24). A follow-up inspection will occur on or after 5/17/24. If the cooler is repaired and holding a temperature of below 41 degrees F, before 5/17/24, please contact Kate at 309-929-0239 for a recheck.
25. A consumer advisory is present on the menu's but an asterisk next to the food items that are allowed to be cooked to order/undercooked, is not available. Provide an asterisk on the menu items allowed to be cooked to order such as eggs, steak and hamburgers and a follow-up inspection will occur on or after 5/17/24.
36. A thermometer is not provided in the Pepsi cooler, located at the wait staff area. Provide a thermometer in all cold holding units in order to monitor the temperature by the next regular inspection.
55. The vent located on the ceiling above the ice machine is heavily soiled. Maintain all ceilings and attached equipment clean. Clean this area by the next regular inspection.

8. Observed a kitchen employee turn off the hand sink faucet with bare hands on two occasions. COS, Informed PIC and employee that a barrier should always be used every time to turn off the water so that hands are not re contaminated before putting gloves on.
43. A bowl was being used as a scoop for the hashbrowns in the sliding cooler. COS, discussed with PIC that a scoop with a handle should be used and the bowl was removed.
49. Some areas of the facility such as the sliding cooler, microwaves and the floors under equipment should be cleaned more frequently so that accumulation of food and dirt don't occur.
