10. *** A wire rack was in the hand sink. *** COS PIC said that the rack was for the dish soap dispenser, and it fell of the wall. He moved the rack so the sink was accessible for handwashing.
39. *** The were soda syrups that were stored directly on the floor. *** COS Boxes were moved off the floor when I informed the manager.
43. *** Utensils for the hot holding side dishes were in water that was above 135 deg F, but the water was visibly soiled. *** COS Manager removed the utensils from the water. The water was changed out for clean water. If the utensils will be stored in hot water, change out water more frequently. I recommend that each hot holding well has its own utensil that is stored in the well with the food so that frequent changing of the water is not needed.
55. *** The floor in the kitchen, warewashing ares, and side room with the ice machine are slippery and have debris buildup in hard to reach corners. *** Clean by next routine inspection. There is a large amount of clutter in the side room with the ice machine that makes cleaning corners difficult. Organizing that room may make cleaning easier.

2. *** PIC has taken the class but was unable to find his license. *** Have a way of verification. A paper certificate or digital one is fine.
8. *** PIC turned off the sink before drying his hands. *** COS Spoke to PIC about proper handwashing techniques. Turn off the knob with a paper towel after drying hands.
10. *** Handwashing sink in the back of the kitchen had no paper towels. *** COS PIC replaced the paper towels. When towel run out, replace them asap.
15. *** Cut of meat in a bin was placed above fries in the walk-in. *** COS PIC moved the meat so it was not above the potatoes. This is a cross contamination risk because potatoes have a lower final cook temperature than all animal products.
23. **** Diced onions, yams, and sauces had no date labeling on them. *** COS, R PIC called the owner and confirmed that all foods were prepared yesterday, and all foods were labeled with the correct date. This is a repeat violation. See attached risk control plan.
39. *** Box of meat was stored on the floor in the walk-in. *** COS PIC moved the box off the floor. Food must be stored 6 in off the floor.

2. *** Person in charge (PIC) was not a Certified Food Protection Manager. The person who is a CFPM was not on site. *** There needs to be a certified food protection manager on site at all times. I will include links to the Tazewell County Health Department info about CFPM classes and the Serv Safe website in the email when I send this report.
10. *** When I washed my hands in the kitchen, there was ice in the hand sink near the cook area, *** COS Hand sinks cannot be used as dump sinks. They must only be used for hand washing. Discussed this when PIC.
10. *** There is only one handwashing sign posted. Other hand sinks have no had washing signs. *** COS Provided hand washing signs for all the sinks so PIC can post them. COS Hand sinks cannot be used as dump sinks. They must only be used for hand washing. Discussed this when PIC.
23. *** Food is made in batches and section off into smaller containers to cool. This facility was marking one container with the date the batch was made, but leaving the others blank. ** COS Informed PIC that all containers need to have date labels, not just one to indicate the day the food was made. The employees put labels on the containers.
