15. In the one door reach in cooler, ready to eat food was on the bottom of the cooler. Hamburger was stored above steak and other ready to eat food. Raw meat was stored on the top shelf, above ready to eat foods. ***COS Reorganized cooler. This is a repeat violation, a Risk Control Plan was created.
22. Observed breaded shrimp, peppers, fries and potatoes at room temperature on top of the prep cooler. Homemade tomato based sauce was found at room temperature next to the hot holding. Person in charge states he puts them up at the start of lunch and then discards them at 4. ***COS Food was put under refrigeration. Time as a control was discussed with person in charge and the establishment was provided with Time as a Control Paperwork. Discontinue practice until Time As A Control paperwork is submitted.
23. No items in the reach in coolers were date marked. The person in charge was able to recall when everything was made. ***COS Person in charge date labeled the items. A date labeling handout was provided.
28. Both bottles of sanitizer on the bar were measuring above 200 ppm. ***COS Bottles were remade to 100ppm.
39. In the storage area in the shed, many food items, including limes, lemons and a bag of bread crumbs were observed sitting directly on the floor. Elevate all food at least 6" off the ground to prevent attracting pests.

58. An Allergen Notice was not posted. ***COS Provided establishment with Allergen Notice.
25. Consumer advisory is not provided on the menus. Correct by 3/18/24. A follow up will occur on or after 3/18/24 to confirm correction.
20. Observed chicken soup in a deep, plastic container in reach in cooler. Soup measured at 74F and had been prepared at 9am (temp taken at 1:30pm). When cooling foods, use shallow pans. Food has 6 hours to fully cool, 2 hours to reach 70F, with the remaining time to get to 41F. ***COS Chicken soup was discarded and education was provided to the person in charge.
15. In reach in cooler, eggs observed above and next to ready to eat foods. Raw steak, ground beef and bones above ready to eat foods. ***COS Reorganized cooler so raw meat is below ready to eat foods.
5. Norovirus cleanup procedures were not able to be provided. ***COS Provided establishment with Norovirus clean up procedures.
