55. Observed multiple ceiling tiles either missing or with water damage. Replace these as often as neccessary

38. Observed front doors are not tight to the top or bottom of the casing. Further, the doors are not tight fitting to each other.
Correct by next routine inspection.
38. Dozens of dead insects observed on the floor beneath the window behind the register.
PIC swept behind register to removed dead insects. Pest control comes out regularly to spray and set traps.
Continue with regular pest control. Increase cleaning as to not have a build up of dead insects.
45. Observed open paper towels being stored in the employee restroom.

Removed and discarded paper towels found in the employee restroom.
47. Observed torn gaskets on several cooler and freezer doors.
Replace gaskets by next routine inspection.
51. Observed handsink in employee restroom to continuously run.
Have handsink repaired by a licensed plumber by next routine inspection.
55. *Outside facing wall of the backroom has at least three area where water is leaking down the inside of the wall. This is causing a white residue which is removable when wiped with a gloved hand. It is also causing a build up of moisture in the back room.
*Ceiling tiles throughout the store are missing or have considerable water damage. Repair all leaks causing the damage and replace missing or damaged ceiling tiles by next routine inspection.
*In the employee restroom, tiles are pulling away from the wall. Repair by next routine inspection.
55. *Observed wall between backroom and hallway to be unfinished drywall with apparent holes.
Finish drywall with latex paint to make the wall smooth, durable and easily cleanable by next routine inspection.
*Floor tiles throughout the store are cracked, damaged or missing. Extensive water damage is apparent in the tiles behind the register.
Replace floor tiles by next routine inspection.
