6. *** Observed an employee eating off of a tray (fry) upon arrival. *** Ensure that food is eaten in a designated area away from prep and service areas.

8. *** Observed an employee's phone to be on a prep line upon arrival. ***COS Employee removed the phone from the prep line.

15. *** Observed an employee with gloves on pick up a fry off the ground to throw away. Employee then went back to the prep area to begin working with food without changing gloves. ***COS Inspector informed employee to remove gloves, wash hands, and don new gloves.

28. *** Observed 2 spray cans of cleaners to be sitting on a storage area above dry food items. ***COS Informed the PIC and she removed the items from the food storage area.

40. *** Observed 2 employees handling food to have wristwatches on. ***COS Informed the PIC and she asked the employees to remove watches.

47. Gasket on True refrigerator is not allowing the door to close tightly and maintains temperature at 41 F. Gasket needs repair to ensure that the product temperature maintains 41 F or below. Gasket appears to be loose at the top or does not have the magnetic lip to pull the door tight.
